Edition originale, tirée à petit nombre. L'hystérie constitue la contribution essentielle de Babinski à la psychiatrie. Joseph Babinski (1857-1932), élève de Charcot, reconsidéra la position de son maître sur l'hystérie. Il avait déjà, devant la société de neurologie, en 1901, exposé sa nouvelle conception ("définition") de l'hystérie, suggérant de remplacer le terme d'hystérie par celui de pithiatisme (phénomènes morbides susceptibles d'être guéris par persuasion). Puis, il développa sa conception dans plusieurs brochures en 1906 et 1907. Le texte proposé ici constitue une synthèse des idées développées par Babinski depuis huit ans. Babinski y précise que, si les résultats de ses recherches l'ont conduit à abandonner la doctrine de son illustre maître sur ce sujet, il n'en conserve pas moins une admiration profonde pour Charcot. Babinski se sert indifféremment de ces deux termes, hystérie et pithiatisme, pour définir cet "état névropathique bien délimité". Ce démantèlement est similaire à celui observé dans les classifications psychiatriques "modernes" (DSM-III, -IV, 5 et ICD-10). "Babinski a incontestablement eu raison de dénoncer les dérives des présentations théâtrales et artificielles faites par Charcot" (Allilaire). Et, comme l'indiquait déjà Guillain, Babinski "a eu hautement raison de conseiller d'examiner tous les malades avec une méthode rigoureuse, d'éviter de les suggestionner, de penser à la simulation. Il a eu raison de mettre en doute ce que l'on appelait, avec trop de facilité, les stigmates de l'hystérie. De plus, il apparaît évident que l'on a porté trop souvent le diagnostic d'hémoptysie, d'hématémèse, d'hématurie hystériques." Babinski semble avoir été moins heureux en réduisant l'hystérie au pithiatisme et en rejetant le rôle de l'émotion, alors que plusieurs concepts de Charcot apparaissent étonnement modernes à la lueur des données récentes de la neurophysiologie et de l'imagerie. Le mérite principal de Babinski est ainsi sa démarche diagnostique rigoureuse, permettant d'exclure formellement une lésion organique du système nerveux ou une simulation, et qui "continue à sous-tendre la pratique neurologique actuelle" (Dérouesné). In a first step, he described many neurological signs in order to clearly distinguish hysterical manifestations from the organic disorders of the central nervous system. The most famous one bears his name, the Babinski sign, an inversion of the plantar cutaneous reflex, testifying to a lesion of the pyramidal tract. In a second step, he defined what remained of hysteria and proposed in 1901 to abandon the term 'hysteria' in favor of the neologism 'pithiatism', defined as a pathologic state resulting in disorders which can be very accurately reproduced by suggestion, and can disappear by persuasion. Babinski therefore retained the exclusive etiological role of suggestion and refuted, unlike Dejerine, the role of emotion. He also sought to separate pithiatism from simulation, but ambiguously he made pithiatics 'semi-malingerers'.
