The gratifying response accorded Current Techniques in Operative Neurosurgery by the neurosurgical community suggested to the editors that a more comprehensive text modeled on the first book would be appropriate. The previous publication arose from the proceedings of a seminar, and the content was limited to constraints inherent in that forum. This new two-volume text addresses the indications, operations, intraoperative considerations, and results of the various forms of surgical intervention within the central and peripheral nervous system. It incorporates revised and updated chapters from the previous book, as well as new sections on anesthetic management, pediatric neurosurgery, surgery of the scalp and skull, head injuries, benign and malignant brain tumors, vascular disorders, functional neurosurgery, surgery of the cranial and peripheral nerves, and surgery of the spinal column and spinal cord. There are also several sections of the book in which alternative surgical approaches to the same problem are presented. These deal with areas in which the decision to use a particular approach may be a difficult one and where it was feit that the presentation of these different points of view could be helpful to the neurosurgeon. Some of these sections are on the surgical management of intraventricular tumors, neoplasms of the pineal region, and acoustic neurinomas. Also included in this volume are discussions of some techniques that are still experimental, such as the stereotactic clipping of aneurysms, or procedures that are of potential importance but have not been widely used within the neurosurgical community, such as neurosurgical endoscopy or the use of intraspinal opioids in the control of pain. In the five years since the publication of the previous text there has been an evolution in our thinking about the different procedures. The majority of those that were selected for inclusion before remain unchanged, at least in regard to their indications and anticipated outcome. Some of the operations, however, have been abandoned by even their most enthusiastic proponents.
