Premi…ère et seule édition, très rare. Les 20 premières pages correspondent à une introduction de Morel sous forme d'une lettre à Ferrus, inspecteur général des asiles d'aliénés de France. Le reste de l'ouvrage, extrait des Annales médico-psychologiques, comporte en tête le titre suivant : "Nouvelles observations sur le goitre et le crétinisme par Mgr Alexis Billet (sic) avec des réflexions de M. Morel". Morel avait observé dans la commune de Rosières-aux-Salines, près de Nancy, une population de crétins myxoedémateux. Mgr Billiet, archevêque de Chambéry mais aussi homme de sciences (fondateur de l'Académie de Savoie) observa des cas similaires dans les vallées alpines. Les deux hommes échangèrent une correspondance, rapportée ici. "C'est ici l'écclésiastique qui est en avance sur le médecin. Il affirme que cette pathologie n'est pas héréditaire. Elle disparaît dans les familles qui quittent la région, le meilleur traitement à son avis." (Hochmann). S'appuyant sur les recherches de Chatin, Mgr Billiet admet le rôle dune carence alimentaire et préconise un apport supplémentaire en iode. Morel, lui, estime que le crétinisme relève de la conjonction de "causes internes essentielles et primitives" (rôle de l'hérédité) et de causes secondaires et accessoires (constitution minéralogique du sol). Ces lettres de Morel préfigurent sa thèse de la dégénerescence ("déviation maladive de l'espèce"). "Cette notion a longuement mûri dans l'esprit de Morel avant qu'il ne la formule dans son traité. Toute la correspondance sur le crétinisme avec l'archevêque de Chambéry, Monseigneur Billiet, en constituait une ébauche deux ans avant son oeuvre maîtresse." (Chebili) Cf. F. Morel introduit la notion de dégénérescence, définie comme une "déviation maladive de l'espèce". Il place l'hérédité au centre de sa théorie. Il distingue des causes prédisposantes ( prédisposition héréditaire) et des causes déterminantes. Il analyse ces causes, individuelles ou générales, morales ou physiques : intoxication alcoolique, haschisch et opium, tabac, saturnisme, substances alimentaires altérées (pellagre, etc.), toxiques, etc. Il analyse aussi l'influence de ces agents selon les races et les climats, et les effets des insuffisances nutritionnelles. Morel décrit l'aggravation de la sévérité ou de la précocité de l'âge de début de certaines affections mentales dans les familles au fil des générations successives ; ce phénomène, décrit (depuis 1910) sous le nom d'anticipation (génétique), après avoir été discrédité au milieu du XXe siècle, a pu être expliqué dans certaines affections par des études de biologie moléculaire. Les conceptions de Morel, exprimées dans cet ouvrage, eurent une influence considérable, jusqu'à la fin du 19ème siècle, non seulement sur les psychiatres français mais aussi sur le monde littéraire et sur la société. Bénédict Auguste Morel (1809-1873) fut l'assistant de Falret. Il succéda à Archambault à l'asile de Maréville, près de Nancy, puis devint médecin-chef à l'asile de Saint-Yon.
