The first edition, dedicated to the Duke of Portland: 'The honour which your Grace has conferred on me, in liberally permitting this new work on gout to be addressed to your notice, is enhanced by the prospect of its affording your Grace that lively interest, which you must feel in every attempt to cure a disease, which has afflicted with deplorable inveteracy your illustrious ancestors, and from the severity of which your own health has not been exempted'. Robert Kinglake, 1765-1842, attracted attention with his controversial writings on gout, in which he advocated a refrigeration treatment. It was based on application of cloths soaked in a mixture of ammonia and cold water. His first papers on the subject appeared in 1801 and 1803 in the Medical and Physical Journal. His views were opposed by many physicians and James Parkinson launched a major attack in 1805, in Observations on the Nature and Cure of the Gout, (1805).

Detailed treatise on gout by Taunton medic Robert Kinglake (1765-1842), who advocates a controversial 'refrigeration treatment', involving the application of cloths soaked in a mixture of ammonia and cold water. Alleviating gouty symptoms was a holy grail of medical endeavour, particularly as so many high-profile people were among the sufferers. One such was the 3rd Duke of Portland, to whom the book is dedicated, who came from a long line of gout-sufferers including his father, who had been so plagued that he developed a treatment powder of his own devising. Kinglake addresses the efficacy of Portland Powders in the present work. Inscribed by the author to the noted agriculturalist John Southey Somerville, 15th Lord Somerville (1765-1819), and from Somerville to Louis Baron de Roll (1750-1813) the Swiss mercenary who formed the regiment of Swiss and Alsatian soldiers which fought for the British Army in the Napoleonic Wars.
