"In 1494 Portugal and Spain struck a deal, drawing on the map a ragged line through the Atlantic Ocean, from the North Pole to the South Pole, then north again from south-to-north through the Pacific Ocean. Spain was to have unto itself all new lands west of the Atlantic line; Portugal the east--each around the world till reaching the line of the other in the Pacific. The parties to this eye-popping arrogance called their agreement the Treaty of Tordesilhas--and had it sanctioned by the Pope. The best of the deal was Portugal's. To their west, they acquired domination over numerous Atlantic islands, and what was eventually to become Brasil--(later amendments to the agreement ensured the line wobbled considerably to the left side of the map.) South and eastwards, they gained domination of almost all of the African continental coastline, the Persian Gulf, the coast of India, Ceylon (Sri Lanka), Myanmar, Malacca and various trading stations in Indonesia as far east as Ternate in the Celebes. They reached Japan and China, and there is some evidence (disputed) they discovered the west of Australia some two hundred years before Cook. There is evidence (also disputed) Açoreano Portuguese were fishing off the Newfoundland, Labrador and Baffin Island coasts prior to Columbus' landing in the Americas in 1492. Spain's take in the deal: the west coast of the Americas, the Philippines--and all the salt water of the Pacific. Over the centuries little bits of Portugal's empire were hacked off by others, but the agonizing conflict that signalled the coming collapse of Portugal's hold on her African colonies began in 1961 with the outbreak of war in Angola and the start of hostilities in Guiné Bissau. It flared in Moçambique in 1964. By the end of 1975 Portugal's entire empire lay in tatters. An Empire Teeters Foray into Portugal's Fascism is not a history of that collapse; it is, rather, the first-hand account of a journalist's curiosity concerning Portugal's African territories in the summer of 1963--when the empire was, indeed, teetering. Government minders--cloak-and-dagger operatives, military brass, and especially the agents of Portugal's political police--all tried to colour their propaganda rosy. By establishing the criteria and algorhythms for the choice of the different methods available for this purpose, defining the diagnostic signs on images and resolving some of the mis conceptions and pitfalls, this book will go a long way to help the reader, particularly those involved in the care of patients in the intensive care units. This book brings together many different methods of investigation and discusses the advantages and limitations of these techniques in different clinical circumstances. Some of the techniques are well established and their usefulness in the intensive care unit is in no doubt. Some of the newer techniques such as PET scanning or NMR imaging have not yet found a defined position of usage in the critically ill patient. There is, however, little doubt that in due course this situation will change.
