The Hitselberger sign (William Hitselberger, neurosurgeon, Los Angeles) describes a loss of sensitivity of the posterior upper section of the external auditory canal in the event of paralysis of the facial nerve (impairment of the intermedius nerve). The symptoms were first described in 1966 by William Hitselberger and William House as an early sign of acoustic neuroma after they noticed a reduced sensitivity of the posterior external auditory canal when rinsing the diseased side with ice water. With high sensitivity, the specificity of the sign for the diagnosis of acoustic neuroma is moderate. William Hitselberger (1930-2014) "was an American otologist remembered for his innovative work, along with Dr. William House, in acoustic neuroma surgery and auditory brainstem implants. Hitselberger sign, lack of sensation in the posterosuperior external auditory canal in patients with vestibular schwannomas, is named in his honor. He received his medical degree from Harvard Medical School in 1956 and interned at the University of Minnesota. He then served as a captain in the U.S. Army Special Forces before returning to Minnesota for a neuropathology fellowship at Mayo Clinic. After completing his neurosurgery training, Hitselberger opened private practice in Los Angeles, and began his collaboration with Dr. House, training the next generation of neuro-otologists and revolutionizing the surgical field. Hitselberger was interred with military honors in Arlington National Cemetery."
