George Ryerson Fowler (1848-1906) reports the "first thoracoplasty". "Fowler received his M.D. from Bellevue Hospital Medical College in 1871. He was an early advocate of Listerism and antiseptic surgery…. He also founded the Brooklyn Red Cross, serving as its first president in 1884, and introduced first-aid and ambulance drills to the New York National Guard. The present report introduced Fowler's operation, a pleurectomy for chronic empyema. Many patients did not benefit from the different radical procedures then used for the treatment of chronic empyema, so Fowler devised his operation of decortication of the visceral pleura: 'An elliptical shaped vertical incision . was made in the axillary line. The soft tissues were retracted from each edge of the incision, the chest wall cleared, and about three and a half inches each of the fifth and sixth ribs removed. There was then revealed a mass of cicatricial tissue which was firmly attached to the chest wall, and through which the sinus passed. This mass was isolated by blunt dissection from its attachments to the chest wall. As the dissection advanced the mass increased in size, until it was found to extend, in a downward direction, to the diaphragm. It was peeled off from the latter until its limit toward the median line was reached'." Rutkow Garrison & Morton No.3179; Rutkow TSp39.
