Édition originale en librairie, rare, de cette thèse remarquable, novatrice, présidée par Bouchard. Elle est basée sur des recherches bactériologiques et des études histopathologiques, les résultats ayant été confrontées avec les données cliniques. L'ouvrage débute par des considérations générales et une étude historique (pp. 11-19) ; Widal rend hommage à Semmelweis. Il démontre que, contrairement à ce que l'on pensait, un seul agent pathogène, le streptocoque (découvert par Pasteur en 1879), suffit à produire les différentes formes cliniques et anatomiques (aiguës ou chroniques) de ''infection puerpérale (à porte d'entrée utérine), la phlegmatia alba dolens, l'érysipèle et le pus des abcès. Il montre notamment que la cause de la phlegmatia "est l'inflammation de la veine par dépôt sur son endothélium du streptococcus pyogenes charrié par le sang. Le caillot se forme consécutivement à cette inflammation de la paroi." De plus, Widal introduit la notion de virulence pour expliquer le caractère polymorphe des lésions : "Ces formes morbides si diverses sont expliquées dans l'immense majorité des cas par les variations dans la virulence du microbe qui est leur cause commune". Cette thèse présente de plus l'intérêt d'être très bien illustrée. Fernand Widal (1862-1929) était alors interne des hôpitaux de Paris, moniteur des travaux pratiques d'anatomie pathologique et suivait les cours de Roux et de Metchnikoff à l'Institut Pasteur. Il devint médecin des hôpitaux de Paris et professeur de pathologie médicale. Il fut membre de l'Académie de médecine et de l'Académie des sciences. Il s'illustra en bactériologie et en néphrologie. Quatre de ses publications ont été retenues par Garrison & Morton (2550, 3783, 5034, 5037, 5090.1).
