Nos Maitres, 40. - Deschiens, édit, Paris 1908, Lichtdruck, 15,5 x 10,5 cm. The challenges of urethroplasty were known to French surgeon Simon Duplay (1836-1924), the father of hypospadias surgery. In 1874 he described tubularization of skin on the penis ventrum to create the neourethra. This maneuver, however, made it difficult to then close shaft skin over the repair, given that preputial skin transfers from the dorsum had not yet been considered. Therefore, to pull the ventral skin edges together, it was necessary to cut a long relaxing incision down the dorsal shaft. Six years later, Duplay pioneered a second approach, in which he committed less ventral skin to the urethroplasty to avoid making a relaxing incision. With this approach, however, there was an insufficient amount of skin to tubularize into a neourethra of adequate diameter. Consequently, Duplay simply left a catheter lying on the strip of epithelium, which he buried under the skin, relying on the healing process to form the remainder of the urethra. Not surprisingly, both of these operations led to fistula development." -cf. Warren Snodgrass.
