J, Walker Downie (1855-1921) - "Who was Dr James Walker Downie and how could he afford such a munificent gift? One of the privileges of any historical writer is the selection of material and while it is all too easy to juxtapose such selected material and imply thatpoi/ hoc ergo propier hoc, I don't think I have done so here, but it is a matter now impossible to prove; in any case it is a logical and plausible tale and does no harm in the telling. The story begins in Vienna in the second half of the 19th century when the internationally famous surgeon Billroth had a favourite pupil Robert Gersuny who became his assistant and subsequently his successor. Gersuny didn't write much but around 1898 be began to describe the injection of paraffin wax, invariably described in the English literature as 'vaseline' as a tissue replacement. He used it to form new testes, to tighten up the tissues around the female urethra to prevent incontinence, but what caught the imagination of the European and American surgeons was its injection to build up the bridge of the nose. These were the days before Ehrlich's discovery of the magic Salvarsan bullet and congenital syphilis with its snub nose was common. Even if the bridge had been flattened by trauma, the stigma of syphilis remained and the demand for soft at body temperature and while this was of little moment and indeed might have been mildly amusing in a testicular replacement, it could be disastrous in a nasal implant and as will be described the material could flow or be pushed through the tissues in a haphazard fashion. There arc of course a wide range of paraffin waxes with higher melting points than paraffine mode. The main difficulty was that they had to be warmed to a temperature high enough to cause tissue burns; otherwise they congealed in the needle and syringe when being injected. Dr Walker Downie's great innovation was to use a paraffin which melted at IO5'F. and was solid at normal body temperature. To prevent it solidifying in the needle while injected he wrote to the British Medical Journal of 8th November 1902 'The proximal half of the ordinary needle of a scrum syringe including the collar is bound round with cotton thread for purposes of insulation. Then a piece of fine platinum wire is wound round the needle and its collar, over the thread, each end of which is soldered to a separate copper wire. When these two copper wires are fixed to the terminals of a storage battery furnished with a rheostat, the platinum wire surrounding the needle may be heated and the temperature raised or lowered as desired'. A layer of moist gauze surrounded the heating wire to provide a 'steam jacket'. In the British Medical Journal of 5th November 1904, two years later, he reported 100 cases .
