"The present article constitutes a report of 172 eases of intrinsic pituitary tumors anti craniopharyngiomas seen at the University of California and Franklin Hospitals (the majority at the former) from 1913 to 1951. The scries consisted of 100 cases of chromophobe adenoma (77 verified by surgery, 12 presumptive diagnoses, and 11 asymptomatic at autopsy), 33 of acromegaly, 23 of craniopharyngioma, 4 of malignant adenoma, 4 of basophil adenoma, and 8 of metastases to the pituitary. The group does not reflect a uniform method of therapy, for the patients have been treated by different members of the attending1 and resident staffs of the two hospitals over this 38 year period, during which many refinements have taken place in the diagnosis and treatment of these lesions. This fact in itself, we believe, lends value to the series.".

"Urea has proved to be a useful agent in lowering intracranial pressure and brain bulk. This agent has generally been administered intravenously in a 30% solution. "The usual response to a major surgical procedure is temporary oliguria which persists for several days. The secretion of antidiuretic hormone (ADH) that produces this oliguria is, in a sense, an "inappropriate" secretion, in that it is not controlled by osmotic stimuli. After surgical manipulation in the sellar and suprasellar areas, however, the su- praoptico-hypophyseal system may be temporarily or permanently damaged, so that a transient or permanent diabetes insipidus results. The coexistence of uncontrolled polyuria with postoperative sodium retention, or with possible excessive sodium loss in patients with prolonged hypopituitarism, may make the management of these patients very difficult. Mixter and Barr in their classical paper in 1934 described the full significance of herniated nucleus pulposus as a cause of pain in the back. Since that time there have been reports of many patients operated upon for lumbar disc pro- trusions.3'5,6'10,11 The potential seriousness of herniated lumbar nucleus pulposus with occasional extradural compression of the cauda equina, paraplegia and block of the cerebral spinal fluid has been well documented.911 However, erosion of a fragment of disc through the dura mater has been mentioned only rarely. One case was described by Dandy in 1942 and 2 cases were reported by Paterson and Gray in 1952. A review of over 2500 surgical cases by O. W. Jones, Jr.4 revealed only 1 such lesion." "Metabolic balance studies, such as those performed by Moore and his co-workers,21 have clarified the aberrations of fluid and electrolyte metabolism that occur during and following surgical procedures. The data thus obtained provided a rational basis for fluid and electrolyte administration during the operative and postoperative periods. There are at least two important reasons for such investigations in patients with intracranial lesions. First, the electrolyte content of intravenous fluids, and possibly of body fluids, affects the cerebrospinal fluid pressure in animals and in man. Secondly, lesions in certain areas of the brain are associated with unusual abnormalities of electrolyte metabolism, e.g., "cerebral salt wasting" and hypernatremia and hyperchloremia. In this paper, the results of metabolic balance studies in 2 patients following craniotomy and in 1 patient after ventriculography will be presented and studies in 5 other patients following craniotomy will be mentioned.
