Neuware -The idea for this Handbook dates back more than a dozen years, to the time when I was commissioned by The Mason and Dixon Lines to develop a handbook that would be useful both to their own people and to their shippers in understanding and applying the basic principles of the then-emerging management science of physical distribution management. Then as now, there were several excellent textbooks in the field. But they were written primarily for classroom use, for persons who would be entering careers in the field at a later date. And there was virtually nothing for the working manager or manager-to-be. Thus we saw the need for a hands-on, practically-oriented guide to physical distribution management, written mainly in non-academic language and supplement ing rather than duplicating the excellent existing coverage of inventory theory, queueing and,other textbook subjects. Bear in mind that the times were quite different, back then. The Na tional Council of Physical Distribution Management (NCPDM) had been in existence barely two years, and had fewer than 200 members. Indeed, there were probably not 100 persons in the country who had the title 'Distribution Manager' after their names. Today, of course, the NCPDM has over 2,000 members and the distribution manager title is widely used and recognized. Writing of the book began at a time when Ted Dangelmayer was burdened with many demands. These demands were from AT&T's own operations, internal suppliers, external suppliers, customers and others looking for a better understanding of the phenomenon of ESD, its impact and, most of all, ways to control and manage it. In a way, this book is a response to these demands by making available a reader friendly document that distills the hard-won experiences of Ted and AT&T. The information and methods in this book have been gained at no small cost and produce results that far exceed eXRenses. There is, however, a caveat: Success will not be obtained unless there is real management commitment. This means management must allocate the necessary resources and provide active support to ensure that training, auditing, reporting, tracking and an aggressive corrective action program all take place successfully. Examples are receiver front-ends, particle detector circuits, etc. Actually, all circuits which require high precision, high speed and low power consumption need analog solutions. High precision also needs low noise. Much has been written already on low noise design and optimization for low noise. Very little is available however if the source is not resistive but capacitive or inductive as is the case with antennas or semiconductor detectors. This book provides design techniques for these types of optimization. This book is thus intended firstly for engineers on senior or graduate level who have already designed their first operational amplifiers and want to go further. It is especially for engineers who do not want just a circuit but the best circuit. Design techniques are given that lead to the best performance within a certain technology. Moreover, this is done for all important technologies such as bipolar, CMOS and BiCMOS. Secondly, this book is intended for engineers who want to understand what they are doing. The design techniques are intended to provide insight. In this way, the design techniques can easily be extended to other circuits as well. The Workshop on Computational Electronics was intended to be a forum for the dis cussion of the state-of-the-art of device simulation. Three major research areas were covered: conventional simulations, based on the drift-diffusion and the hydrodynamic models; Monte Carlo methods and other techniques for the solution of the Boltzmann transport equation; and computational approaches to quantum transport which are relevant to novel devices based on quantum interference and resonant tunneling phenomena. Our goal was to bring together researchers from various disciplines that contribute to the advancement of device simulation. These include Computer Sci ence, Electrical Engineering, Applied Physics and Applied Mathematics. The recommended dose, its potential toxicity, the side effects of under-or over-dialysis as well as its efficacy have been the subject of very few studies. The high mortality rate associated with the treatment may raise a few eyebrows. That it is a life-saving modality of treatment is undoubtedly true for more than 100,000 patients in the United States and for more than a million patients world wide. Because dialysis has extended the lives of many people by a variable period of time, most nephrologists have 'rested on their laurels' and did not vigorously pursue studies to optimize these treatments. But facts have a way of intruding in all our lives and the facts are that the overall mortality rate of dialysis patients in the United States is rising and stands close to 25% per year and is closer to 33% per year for patients between the ages of 65 and 74 (2). We believe that, by following the subjeets eovered by the Yearbook in the broad field of nephrology, the praetieing nephrologist and nephrologist-in-training ean stay abreast of a number of rapidly advaneing fields. To improve the quality of the Yearbook, we have appointed an Editorial Board to provfde suggestions for topies and authors. Thus, we have invited 58 outstanding nephrologists from around the world (the list of them is included in this issue of the Yearbook) to identify annually potential topies for the Yearbook. The reply was exeel lent and the contribution quite outstanding. Thus, for the 1992 Yearbook, we reeeived more than 150 suggestions. A number of them will be included in the next issue if they continue to be viewed as eurrent and important. We also invite readers to suggest topies for upeoming issues of the Yearbook. The present issue, the 1991 Yearbook, represents an improvement over previous volumes. We expeet this trend to be maintained in future years. As in previous issues, all ehapters provide a eomplete, aeeurate and up-to-date list of important references. Neuware - Even the most casual reader leafing through the pages of this book will quickly realize that it is not the thoughts of one indi vidual set down on paper, but rather the synergism of many people. Herbert Goeler of American Cyanamid and Keith Slater of Johns-Manville, the latter now retired, are certainly the brightest stars in my particular customer service galaxy. They have been well-springs of information, thinkers and thought starters, friends and fellow-believers, for almost two decades. Without the encouragement and inspiration they gave me, this book would be much shorter and far less relevant. In 1984, my firm presented Herb and Keith with our special 'Patron Saint' Award; it doesn't say nearly enough for what they have meant to me personally and, through me, to the field of customer service management where I disseminate ideas and guidance as an editor and teacher. Some years ago, the American Management Associations asked me to chair a series of seminars on Customer Service Management in the U. S. and Canada. Through this activity I met many wonderful people who in many cases taught me more than I taught them.

It represents the fifth generation of books on this subject written by its author. Significant additions and revisions have been made in this edition. Chapters 2 and 3 contain new material intended to assist the engineer in understanding factors affecting the time-dependent properties of the reinforcement and concrete used in prestressing concrete, as well as to facilitate the evaluation of their effects on prestress loss and deflection. Flexural strength, shear strength, and bond of prestressed concrete members were treated in a single chapter in the of flexural strength has third edition. Now, in the fourth edition, the treatment been expanded, with more emphasis on strain compatibility, and placed in Chapter 5 which is devoted to this subject alone. Chapter 6 of this edition, on flexural-shear strength, torsional strength, and bond of prestressed reinforce ment, was expanded to include discussions of Compression Field Theory and torsion that were not treated in the earlier editions. In similar fashion, expanded discussions of loss of prestress, deflection, and partial prestressing now are presented separately, in Chapter 7. Minor additions and revisions have been made to the material contained in the remaining chapters with the exception of xv xvi I PREFACE Chapter 17. This chapter, which is devoted to construction considerations, has important new material on constructibility and tolerances as related to prestressed concrete.

The advan tages of precasting include excellent quality control, economical large scale production, improved construction productivity (especially in adverse weather conditions) and immediate structure availability. These advantages have been recognized for precast concrete raft pavement units (raft units) since their introduction in the 1930s. In the last ten years there has been a considerable increase in the use ofraft units, especially in their range of applications, their analysis and their design. However, the description of these developments has been published in academicjournals and conference proceedings which are not readily available to practising raft unit pavement design engineers. Pavement design engineers are underincreasingpressure to produce raft unit designs that are inexpensive, long lasting and able to allow reorganization to accommodate changing use and uncertainty offuture loading requirements. This is the first book devoted to raft unit pavements, and will become a standard work of reference.
