That it is comprised of occasion, speaker, and topic is a gross oversimplification. It also includes challenge, anxiety, emotion, fear, responsibility, faults of memory, and instants of pride. Out of the circumstances arise an increase in heart rate, a change in blood pressure, an abnormal pattern of breathing, a noticeable build up in perspiration, and an ongoing evaluation. For students this may be merely a grade or perhaps a series of evaluative remarks, possibly addressed both to the speaker and the other participants, the audience. It may entail a replaying of a record of the speech, indeed a videotape. Most important is the lasting impression that remains with all of the participants. What of the vocabulary of the speaker under the circumstances of the speech situation This speaker - in the major portions of this work we may say, 'this young man' - has spent time seeking an appropriate topic. He has outlined a composition around a central idea or thesis. He has marshaled evidence, details. He has framed an opening paragraph. He has been admonished not to give an essay, but to strive for audience contact, interpersonal communication. He makes his audible approach through his vocabulary and accompanying phonology. Under the tension, the speaker repeats; he adds meaningless vocalizations in periods that might logically be pauses. There are slips of the tongue. Typically, after reading the recommended materials, the student returns to request more detailed accounts of how clinicians 'think' about particular cases. The general lack of such materials in the behavior therapy Iiterature led to the formation of the present volume. Throughout much of the behavior therapy literature, one gets the impression that most cases seen present circumscribed and straight fmward psychological problems. In my experience, such cases are rare. Accordingly, the present volume was designed to cover more complex problemssuch as sociopathy and paranoid personality. Thesedisorders are rarely discussed in the behavior therapy Iiterature but nonetheless seem to appear regularly in clinical settings. The cases presented in this book are descriptions of patients seen clinically by the editor or by the contributors. Work an this text began while I was a faculty member at Vanderbilt University and took several years to complete. As the contributors would attest, the task I set out for them was atypical and often difficult. The complexities involved in articulating how one conceptualizes a case are numerous and may help to explain why there are so few sources available on case formulation. Like volume IX, this one focuses on one of the subgroups to which we are committed--the neonates' special fluid and electrolyte requirements. This volume has more on blood pressure and renal function and looks at the hormonal regulators. There is greater depth about intoxications and nephrotic agents, congenital disorders and mineral metabolism. The exchanges were stimulating and the controversies were brought out without need of much of my usual prodding. At Julie Ingelfinger's suggestion, at the end of each panel discussion I have added a comment to highlight the main points as I see them. Otherwise, the format remains as in past editions: the papers given related to the four major topic areas, each followed by panel and registrant discussion. Although the transcription is almost verbatim, you will not find the names of the discussants, purposely omitted to ease my editorial work and to encourage everyone to speak candidly. Some of the questions and answers are those submitted to the panelists after the sessions, incorporated here by request. Also, frequent references are made to others' work but their names have been omitted.

It is undoubtedly difficult to determine, even in normal women, the behavior of renal hemodynamics throughout gestation, since the fear of impairing a new life (i.e., the fetus's life) will limit, for ethical reasons, the use or the frequent repetition of diagnostic tests on the mother. On the other hand, the study of complicated pregnancy even for diagnostic purposes (for planning adequate treatment), except in a few countries that are known for the advanced health education of the population, has to face serious difficulties. First of all, pregnant women usually seek the help of an obstetrician when gestation is already in an advanced stage. This makes it difficult to determine when and how asymptomatic signs of any disease discovered during pregnancy have first occurred. A second difficulty is that frequently the patient does not know whether a given disease has preceded pregnancy. Pregnancy is a condition of young women, and a young woman frequently has never seen a physician; thus, no urine analysis or blood tests have been performed before the gestation. Not infrequently, even blood pressure has never been measured. This will make it difficult to classify hypertension discovered in late pregnancy as pregnancy-induced hypertension or as chronic hypertension in pregnancy.

Gains with cytotoxic chemotherapy have been much more modest. Of the approximately 900,000 newly diagnosed cases of cancer each year, 50010 result in death of the patient. The primary cause of these deaths is metastasis. Although the term metastasis was first coined by Recamier in 1829, only in the past ten years have there been intensive scientific investigations into the mechanisms by which tumor cells metastasize. What has emerged is a complex process of host-tumor cell interactions which has been termed the metastatic cascade. Due to the complexity of the metastatic process, the study of metastasis is multifaceted and involves elements of such areas as differentiation, en zymology, genetics, hematology, immunology, membrane biochemistry and molecular biology. The major objectives of this book were to present the most recent advances in our understanding of how tumor cells metastasize to secondary sites by the leading experts in the biology of tumor invasion and metastasis. We hope that this book will lead to new concepts for the treatment of subclinical metastatic cancer. The chapters in this book address both the basic science of metastasis and potential clinical therapies directed toward interruption of the metastatic cascade or toward eradication of subclinical metastases. Many relevant topics have been omitted due to space considerations and thus the topics included reflect the prej udices of the editors.
