Since that auspici ous event, nine international symposia on the role of taurine in biology have taken place. The locations for these meetings have been Tucson (two…times), Rome, Philadelphia, Tokyo, Vancouver, Mexico City, Helsinki, and Florence. In 1977, due to the large number of scientists in Japan who were interested in the role of this unique amino acid in biological systems, we organized the Japanese Research Society on Sulfur Amino Acids with the encouragement and financial assistance of the Taisho Pharmaceutical Co., Ltd (Tokyo). Annual meetings have been held, and the membership has expanded from 78 to 414 in 1987; the number of presentations has increased during this time span from 29 to 74. The symposium in Tokyo in 1982, 'Sulfur Amino Acids, Biochemical and Clinical Aspects' [1], was held to celebrate the 5th Annual Meeting of our Society. I would like to emphasize that in Japan we have an active Research Society especially directed to the study of sulfur amino acids. We have published our own semi-annual journal entitled Sulfur Amino Acids. Our society is an inter disciplinary research society since taurine is a highly diversified compound that interconnects physiology, biochemistry, pharmacology, nutrition, and medicine. One exciting fringe benefit of taurine research and the society has been the fostering of contacts with distinguished scientists from many varied medical fields.

Despite these screening efforts, carcinoma of the cervix remain…s one of the more common malignancies in women and it is the leading cause of cancer death in many countries in the western hemisphere. It is estimated that in 1986 there will still be 14,000 new cases of invasive cancer, with 6,800 deaths in the United States alone. Unfortunately, many of these patients present with advanced disease, posing difficult management problems for the clinician responsible for their care. The treatment of early stage invasive carcinoma of the cervix (lesions confined to the cervix and vagina) remains either radical surgery, radical radiation therapy or a combination thereof This approach is extraordinarily effective in the vast majority of patients. However, there remains a subset of patients with early stage disease that are at high risk for recurrence. Dr Kjorstad (Chapter 2) has identified adenocarcinomas and adenosqua mous carcinomas as having a particularly poor prognosis. In addition, patients with more than three positive lymph nodes or with involvement of lymph nodes outside of the pelvis have a very poor prognosis. He has iden tified the CEA as a potentially predictive marker for these patients with poor prognosis, especially in patients with adenocarcinomas.

Put simply, we noticed that many VLSI design tools looked 'alike'. That is, at least at the overall software architecture level, the algorithms and data structures required to solve problem…X looked much like those required to solve problem X'. Unfortunately, this resemblance is often of little help in actually writing the software for problem X' given the software for problem X. In the VLSI CAD world, technology changes rapidly enough that design software must continually strive to keep up. And of course, VLSI design software, and engineering design software in general, is often exquisitely sensitive to some aspects of the domain (technology) in which it operates. Modest changes in functionality have an unfortunate tendency to require substantial (and time-consuming) internal software modifications. Now, observing that large engineering software systems are technology dependent is not particularly clever. However, we believe that our approach to xiv Preface dealing with this problem took an interesting new direction. We chose to investigate the extent to which automatic programming ideas cold be used to synthesize such software systems from high-level specifications. This book is one of the results of that effort.

In time, experience led all of us from the several disciplines involved in this enterprise to the conclusion that the crude diag nostic techniques available to us circa 1945-1965 had garnered us little valid information upon which to base such complex, differential diagnostic decisions. It now is gratifying to look back and review the remarkable progress that has occurred in the field of clinical neuropsychology in the four decades since I was a graduate student. In the late 1940s such pioneers as Ward Halstead, Alexander Luria, George Yacorzynski, Hans-Lukas Teuber, and Arthur Benton already were involved in clinical studies that, by the late 1960s, would markedly have improved the quality of clinical practice. However, the only psychological tests that the clinical psychologist of my immediate post-Second World War generation had as aids for the diagnosis of neurologically based conditions involving cognitive deficit were such old standbys as the Wechsler Bellevue, Rorschach, Draw A Person, Bender Gestalt, and Graham Kendall Memory for Designs Test.

Even in the 'Scuola Salernitana', the oldest medieval medical school in Western Europe, about 1000 years ago it was taught how to improve urine output. The list of known 'diuretica' included herbs, plants, roots, vegeta…bles, in particular asparagus, fennel and carrot. The first diuretic drugs, however, were mercurial compounds. Thus, calomel, mercurous chloride, was initially used as a diuretic in the sixteenth century by Paracelsus, being one of the ingredients of the so-called 'Guy's Hospital pill'. But calomel had a cathartic effect so that it was replaced by organic mercurial compounds. These diuretics were clearly toxic. After the discovery of the car bonic anhydrase, in the early 1930s, and the introduction of sulfanilamide as a chemotherapeutic agent, it was observed that this drug was inhibiting carbonic anhydrase in vitro and urinary acidification in vivo thereby causing metabolic acidosis; urine output, however, appeared to increase. Subsequent studies led to the synthesis of more potent analogs, in particular acetazolamide. Studies on car bonic anhydrase inhibitors led to the synthesis of benzothiadiazides which disclosed much less inactivating action on carbonic anhydrase and much more diuretic effect through an inhibition of tubular transport of sodium and chloride. Chlorothiazide was the first member of this class of diuretics. Thiazides are still used in clinical practice.

Congestive heart failure also is one of the commonest causes for hospital admissions accounting for over 5 million hospital days per year. Despite the early recogn…ition of this condition and active medical research into both mechanisms and therapy, prognosis continues to remain dismal wi th less than a 50% expected five year survival. In the last decade we have seen many new medical and therapeutic options for patients with congestive heart failure which extend beyond the use of bed rest, sodium restriction, digitalis and diuretics. These include vasodilators of a variety of types including the angiotensin conventional enzyme (ACE) inhibitors. Also, many new inotropes are under active investigation both in oral and intravenous forms. In March of 1984 a survey of over 5000 physicians was performed under the auspices of the American Heart Association (reported in: JAOC 8:966, 1986). That survey showed that there was no universally accepted defini tion for congestive heart fai lure and that a wide spectrum of diagnostic cri teria for this common condi tion existed even among academic cardiologists. There was no clear standard as to even the mos t bas ic treatment of conges t i ve heart fai lure. For example, exercise restriction was recommended by 19% of physicians, 31% recommended no change in activity, and 50% either light exercise or an exercise conditioning program.
