They are the most frequently measured endocrine tests. The sensitive thyrotropin (TSH) assay reflects thyroid hormone feedback to the pituitary, and is diagnostic of both thyroid h- mone excess as well as deficiency. The log¿linear relationship between serum TSH and thyroxine concentrations means that small changes in serum thyroxine are amplified by changes in serum TSH. The availability of the sensitive TSH assay in essentially all clinical laboratories has improved and simplified the assessment of thyroid function for the diagnosis of thyroid disease and to monitor treatment. Serum free thyroxine and thyrotropin concentrations, as well as other thyroid tests, can be measured utilizing an automated immunoassay platform that provides rapid and accurate results. This simplified approach to thyroid assessment, often requ- ing only a serum TSH measurement, and rapid availability of the thyroid function tests results, has expanded the scope of thyroid testing and clinicians ordering and interpretingth yroid tests. There remain, however, many challenges in selecting the appropriate thyroid function test to order, the correct interpretation of results, and applying these results to the diagnosis and management of thyroid diseases. It is especially important to be aware of limitations of thyroid function tests, as well as special clinical c- cumstances that can influence thyroid function measurements. The goal of this work is to present what experts in the eld feel is the current knowledge and future direction of breast cancer prevention and early detection. We begin Part I of the book with a review of risk factors, both genetic and environmental. We next review progress in the use of chemoprevention. Notably, chemoprevention risk reduction studies have led to FDA approval of two medications which measurably reduce disease incidence among women at increased risk, although with some risk of treatment related side effects. Newer agents in the pipeline, which may also reduce risk among normal risk women, are also discussed. Surgical risk reducing strategies complete the section on prevention, including both the bene ts and downsides to this more aggressive approach. Even with aggressive prevention strategies, some women will develop breast cancer. For these women, early detection is critical to minimize disease spread and maximize long term survival. The hypoxic zone, an area of low dissolved oxygen that cannot s- port marine life, generally manifests itself in the spring. Since marine species either die or ee the hypoxic zone, the spread of hypoxia reduces the available habitat for marine species, which are important for the ecosystem as well as commercial and recreational shing in the Gulf. Since 2001, the hypoxic zone has averaged 2 1 16,500 km during its peak summer months , an area slightly larger than the state 2 2 of Connecticut, and ranged from a low of 8,500 km to a high of 22,000 km . To address the hypoxia problem, the Mississippi River/Gulf of Mexico Watershed Nutrient Task Force (or Task Force) was formed to bring together represen- tives from federal agencies, states, and tribes to consider options for responding to hypoxia.
