Neuware -The past decade has witnessed a considerable upsurge of clinical and research interest in the problems of developmentally and physically the fields of psychology, disabled persons. Indeed, professionals from special education, rehabilitation, psychiatry, and social work have all directed attention to the development and implementation of evaluation and remedial programs. The heightened activity in this area is in part a result of early assessment research with these popUlations, which provided preliminary evidence of difficulties in social and emotional adjustment in many disabled individuals. In response to these findings, many intervention efforts have been implemented to deal with these issues and improve the life situation of the disabled. Also, there were indications that developmentally and physically disabled children often were deprived of adequate educations. As a result, legal and legislative initiatives have been enacted in recent years to make public education available to all students irrespective of disability. Central to all therapeutic and psychoeducational endeavors with the disabled is psychological evaluation. Only through careful and com prehensive psychological evaluation can areas of deficit as well as strengths be identified. Once treatment targets and goals have been ascertained, psychological evaluation is needed to determine the efficacy of intervention strategies. However, the field of psychological evalua tion, in general, and with disabled persons, in particular, has become highly specialized. It is an attempt to establish a critical mass of oncology literature covering virtually all oncology topics, revised frequently to keep the coverage up to date, easily available on a single library shelf or by a single personal subscription. Neuware - During the past decade, there has been a renaissance of interest in the use of peritoneal dialysis as a primary dialytic modality for the treatment of children with end stage renal disease (ESRD). The development of the technique of continuous ambulatory peritoneal dialysis (CAPD) and continuous cycling peritoneal dialysis (CCPD) has markedly changed the approach to children requiring dialytic therapy. The availability of these techniques has facilitated prolonged dialysis in infants and has for the first time given pediatric nephro logists in many areas of the world an opportunity to consider dialysis in chil dren afflicted with ESRD. I have enlisted the collaboration of colleagues from Europe, South America, Canada, and the United States in compiling this multidisciplinary text, which hopefully contains the most up-to-date, comprehensive information regarding the use of CAPD/CCPD in children. It is my hope that every nephrologist (pediatric and adult); nephrology nurse (pediatric and adult); nephrology tech nician, or allied health professional dealing with children who require these therapeutic modalities will be able to resolve immediately any confounding clinical or technical issues that arise by using the information contained in this text. Demographic data on the use of CAPD/CCPD in children in Europe is provided from the EDTA Registry and in the United States from the National Peritoneal Dialysis Registry. The particular problems encountered in the use xiii xiv Preface of CAPD in children in developing countries is detailed by Dr. Grunberg and his colleagues in Uruguay.

Buch. Zustand: Neu. Neuware - The chapters in this book are those presented at the Second International Symposium on Fundamental Problems in Breast Cancer held at Banff, Alberta, April 26-29, 1986. This is, therefore, the second volume in the series of symposia held in the Canadian Rockies. The aim of these symposia is to provide a supportive atmosphere for the development of new concepts as well as for the presentation of high quality scientific data. This is reflected in the chapters presented here. Many of the chapters put forward new hypotheses which will be tested in the clinic or laboratory in the next few years. In choosing the subjects for discourse, we preferred to tackle areas where controversy existed, and in this book we have grouped the chapters around discussion themes. The relative merits of mammography and physical examination in the detection of small lesions within the breast and the natural history of these lesions were important topics at the symposium, and the results of early detection studies will have a major effect on health care costs. We have devoted a large section of this book to this subject. The mechanism of growth and response of endocrine sensitive tumors and current concepts of the statistics and cost-benefit assessment of adjuvant chemotherapy trials comprise two other sections of the book where original and stimulating ideas were presented.

Andreucci of keeping alive patients in terminal chronic Initially created with the purpose renal failure, dialysis has undergone improvements in methodology, and its final goal has become complete health rehabilitation and optimization of the quality of life of chronic dialysis patients. To achieve this, many investigators have attempted to increase dialysis efficiency and at the same time shorten dialysis time. Their main concern was, obviously, patient safety: the Latin proverb 'primum non nocere' is still valid all over the world. Thus, when clinical observations of the first patients on regular dialysis therapy suggested an inverse relationship between duration of dialysis sessions and severity of peripheral neuropathy, long and frequent dialysis sessions were considered the only way to prevent the catastrophic consequences of nerve damage and underdialysis syndrome. It was then, in 1971, when dialysis duration was 8- 12 hours per session, that Vincenzo Cambi started a 'short dialysis' trial, i. e. , 4 hours 3 times weekly or 3 hours every second day. For the first time, dialysis was shortened from 24-36 hours weekly to 10. 5-12 hours weekly [1, 2]. In 1971 I was still at the Parma University Hospital. We had both just returned from the United States, and Dr. Cambi was responsible for the dia lysis unit.

Buch. Zustand: Neu. Neuware - The last decade has generated a multitude of studies using in vitro model systems to explore growth and differentiation of the nervous system. Although the findings have been exciting and have revealed unique properties of neural cells, considerable concern continues to be expressed regarding the significance of in vitro findings in terms of their applicability to in vivo biological events. To examine this issue further, a group of scientists pre sented and discussed their findings at a conference sponsored by the Institute of Developmental Neuroscience and Aging held in Crete, Greece, 26-29 May 1985. The conference was cosponsored by the University of Crete and was generously supported by the Ministry of Research and Technology of Greece, Tourism Organization of Greece, and also Sandoz and FIDIA. The Directors of the Institute of Developmental Neuroscience and Aging are indebted to these Institutions for their support. For the success of this conference, the Directors owe much to Drs. Eleni Fleischer-Lambropoulos and Yiannis Tsouderos, who spent countless hours in making arrangements so that the participants would have not only a scientific, but also a unique cultural, experience. Several chapters of this book focus on the complex phenomena of neurogenesis and gliogenesis, and the modulation of neuronal differentiation. The concept that neuronal differentiation has both genetic and epigenetic components is documented by elegant studies using both in vitro cultured cells and neurons transplanted in vivo.

The emphasis in this volume is to present a series of papers on areas of high clinical relevance in malignant diseases of the gut. As in the first and second volumes of this series, authors have been selected for their expertise and national and international prominence in their fields. This volume is organized so that papers explaining basic science pro spec tives proceed those dealing with clinical aspects of gastrointestinal cancer. It is clear that in many instances advances gastrointestinal carcinoma cannot be effectively treated if ' cure' is the desired goal. When faced with poorly treatable diseases it is obviously important to look toward the causes and prevention of these illnesses. For this reason, there are several chapters in this volume that examine the issue of carcinogenesis of gastrointestinal cancer. Likewise, in diseases that are poorly treatable in advances stages, one is interested in early detection. Thus, early screening of populations becomes important and is dealt with in three papers in this volume. Chapt ers on treatment explore innovative approaches to therapy of gastrointesti nal cancer. Second-look surgery with resection, arterial perfusion with che motherapy, adjuvant therapy and neoadjuvant therapy are all addressed in various chapters in this volume. Finally, four chapters deal with unusual problems in gastrointestinal cancer. These papers include discussions ofpri mary hepatobillary cancer, lymphoma of the gut, and gastrointestinal endo crine tumors.
