Admiral S.M Nanda, PVSM, AVSM Chief of Naval Staff during 1971 The book was first published in 1987 and was also the first ever book on Indian Navy's role in the 1971 Indo-Pak war leading to liberation of Bangladesh. The book gives adequate coverage to the Pakistani account of naval war and hence is a very representative book on the event. The edition one of the book has been referenced in most of the articles on the subject and continues to be the debated. This edition has few corrections and graphics but the essence of the content has been preserved.

The Operational Safe…ty (OS) of Industrial Systems is today a true engineering discipline, applied in all the different phases of the life of an industrial system, from its conception to its decommissioning, going through the stages of development and operation. In a broad sense, the Operational Safety of Systems can be defined as 'Science of Failures'. It thus includes knowledge, assessment, prediction, measurement, and control of system failures. In a strict sense, the Operational Safety of Systems is the ability of a system to successfully accomplish the mission for which it was designed, without the occurrence of events with undesirable consequences not only for the components of the system but also the operators, the general public and environment with which the system is in interaction. The objective of the present work is to present the basic concepts and probabilistic methods applied in the different phases of the life of an industrial system to provide an adequate Operational Safety. For this, it begins by presenting some fundamental concepts, deepening in the main component concepts of OS: Reliability, Availability, Maintainability and Security. Next, the use of probabilities is discussed, as well as their most significant laws within the application fields of OS and formalizing the concept of risk. The allocation methods and the assessment methods of the safety of an industrial system are then presented and discussed. Finally, it is proposed a rational procedure for the safety analysis of systems, and ways of using this procedure to the design of systems.

Be it a two man swimmer delivery vehicle or deep water Kilo's and HDW's or blue water Charlie I and Akula II class SSGN's or strategic Arihant class SSBN's, the navy has operated the entire spectrum of submarine types. The grit and determination of the submariners is not just limited to fighting but also to build their own machines and fight. The submarine arm is also a bit of mystery and usually finds mentions in few pages in naval books or makes headlines during accidents. The book traces the origin and growth of the submarine arm of the Indian navy. The book has been written in consultation with Cmde Arun Kumar, AVSM, NM (Retd.) who has served 28 years in the submarine arm and has headed critical submarine upgrade and procurement plans. In his twenty eight years of submarine career, he held various staff and command appointments.
