Biomaterials in implants: The principal classes of biomaterials are reviewed, and the requirements that must be met for various indications are discussed. Techniques of cholesteatoma surgery, and recent controversies surrounding them, are presented in this chapter. Surgical management is significantly influenced by knowledge of the various forms of cholesteatoma and their pathogenesis. This discussion takes into account the results of recent studies in molecular biology, with a focus on tailoring cholesteatoma surgery to the individual patient. Stapes surgery has a predictably high success rate in experienced hands with a very low incidence of complications. In this chapter, numerous technical refinements are presented, giving special attention to improved instrumentation and various laser techniques. Partially and completely implantable hearing aids have developed at a rapid pace in recent years; the chapter here, written by a pioneer in the area, presents the types of hearing aids currently in use. With convincing clarity, a new nomenclature for active hearing aids is suggested, based not on the implantation site but on the function that the device replaces. The indications for hearing aid implantation and the clinical results that have been reported so far are reviewed in this chapter.
