Cet article fondamental de Laennec, non illustré, occupe les pages 46-61 de ce volume. Ce volume (qui s'étend de l'article Amulette à l'article Bandelette) fait partie du célèbre dictionnaire encyclopédique en 60 volumes parus chez Panckoucke (1812-1822). Cet "article" précurseur de Laennec, qui ne fit pas l'objet d'un tirage à part et qui ne fut pas réédité, est régulièrement cité dans les articles et ouvrages consacrés à l'histoire de l'anatomie pathologique et de la cancérologie. Laennec se révèle un pionnier de l'anatomie pathologique moderne et de la méthode anatomo-clinique. Il définit l'anatomie pathologique comme une "science qui a pour but la connaissance des altérations visibles que l'état de maladie produit dans les organes". Il estime que l'anatomie pathologique couplée à "l'observation des symptômes ou des altérations de fonctions" est "le guide le plus sûr pour le diagnostic médical". En fonction de la présence ou non de lésion, Laennec distingue les maladies organiques et les maladies nerveuses (fonctionnelles). Il propose une classification des altérations organiques, qui peuvent être isolées ou associées entre elles, "combinées". Parmi les "tissus accidentels qui n'ont pas d'homologues parmi les tissus naturels", il distingue, à côté des tubercules, le squirre, les encéphaloïdes et les mélanoses ; il expose les caractères de ces différents tissus à qui il reconnaît une influence générale sur tout l'organisme. Laennec propose ainsi une classification des cancers basée sur l'aspect macroscopique des tissus tumoraux et les caractères du stroma; il y intègre le mélanome (dénommé mélanose) dont il donna la première description scientifique en 1806. Laennec avait conçu le projet de rassembler ses recherches en anatomie pathologique dans un traité, dont seule une partie fut rédigée et publiée longtemps après sa mort, en 1884. (Laennec was the first to recognize melanoma as a distinct type of cancer.) His descriptions of the gross features of these types are exceedingly accurate. It is apparent that his autopsy experience with cancer was extensive. Laennec's premature death prevented him from writing a treatise on general pathological anatomy which he had planned.
