The Boeing B-17 Flying Fortress is a four-engined heavy bomber developed in the 1930s for the United States Army Air Corps (USAAC). Relatively fast and high-flying for a bomber of its era, the B-17 was used primarily in the European Theater of Operations and dropped more bombs than any other aircraft during World War II. It is the third-most produced bomber of all time, behind the four-engined Consolidated B-24 Liberator and the multirole, twin-engined Junkers Ju 88. It was also employed as a transport, antisubmarine aircraft, drone controller, and search-and-rescue aircraft. In a USAAC competition, Boeing's prototype Model 299/XB-17 outperformed two other entries but crashed, losing the initial 200-bomber contract to the Douglas B-18 Bolo. Still, the Air Corps ordered 13 more B-17s for further evaluation, then introduced it into service in 1938. The B-17 evolved through numerous design advances but from its inception, the USAAC (later, the USAAF) promoted the aircraft as a strategic weapon. It was a fast, high-flying, long-range bomber with heavy defensive armament at the expense of bombload. It developed a reputation for toughness based upon stories and photos of badly damaged B-17s safely returning to base. The B-17 saw early action in the Pacific War, where it conducted raids against Japanese shipping and airfields. But it was primarily employed by the USAAF in the daylight strategic bombing campaign over Europe. "Pilot Training Manual for the B-17 Flying Fortress", which was issued to B-17 pilots during World War II and was considered to be the "textbook of the B-17". The manual begins with a brief history of the B-17, and then goes on to explain to the pilot the duties and responsibilities of each of his crew members. Following this is a series of topics dealing with the characteristics and operation of the B-17 itself and how the crew should handle various emergency situations. From the Foreword: This manual is the text for your training as a B-17 pilot and airplane commander. The Air Forces' most experienced training and supervisory personnel have collaborated to make it a complete exposition of what your pilot duties are, how each duty will be performed, and why it must be performed in the manner prescribed. Low-level waste (LLW) is nuclear waste that does not fit into the categorical definitions for intermediate-level waste (ILW), high-level waste (HLW), spent nuclear fuel (SNF), transuranic waste (TRU), or certain byproduct materials known as 11e(2) wastes, such as uranium mill tailings. In essence, it is a definition by exclusion, and LLW is that category of radioactive wastes that do not fit into the other categories. If LLW is mixed with hazardous wastes, then it has a special status as mixed low-level waste (MLLW) and must satisfy treatment, storage, and disposal regulations both as LLW and as hazardous waste. While the bulk of LLW is not highly radioactive, the definition of LLW does not include references to its activity, and some LLW may be quite radioactive, as in the case of radioactive sources used in industry and medicine. The definition of low-level waste is set by the nuclear regulators of individual countries, though the International Atomic Energy Agency (IAEA) provides recommendations. The exact features of a system of conduct of operations may be different at different activities. The common feature is a formality of operations which will vary in form and degree depending on the conditions and hazards of the activity. The most intensive application of conduct of operations would be found at the most hazardous activities subject to repetitive types of evolutions. Operational formality is a structured and systematic way of performing work. It is not simply a listing of functional areas, but rather a mind set, or way of doing business. A comprehensive program of operational formality should provide detailed guidance for performing essential elements of operations, such as: shift routines, communications, control of equipment, lockouts and tagouts, logkeeping, operations procedures, and equipment labeling. Data are also presented on occupational injuries and fatalities in all activities of the Atomic Energy Commission since the beginning of the AEC as a civilian agency, Jan 1, 1947, through Dec. 30, 1958. Narrative descriptions are included on all fatal accidents during 1957 and 1958. The contents are: Industrial Injury Rates, AEC Fatal Accident Experience, A Summary of Accidents Involving Fatalities in Atomic Energy Activities, 1957-58, Lost Time Injuries from Radiation, and A summary of Accidents Involving Radioactive Materials in Atomic Energy Activities, 1957-58. Where comparisons are made, the latest data on injuries published by the National Safety Council were used. The Commission's injury rates, taken as a whole, including accidents involving nuclear radiation, compare very favorably with the best rates in the National Safety Council's annual report. The over-all AEC injury frequency rate for several years has been second or third from the top. In most cases of strains and sprains, workers were lifting improperly or failed to obtain proper assistance. Over 90 per cent of the fracture resulted either from workers falling from a different level or from falling objects striking the workers. The fatality rate was attributed to increased construction activity. It was noted that at all times the rate (deaths per 100,000 employees) had been below the average industrial occupational fatality rate reported by the National Safety Council.
