Collaborative Drug Therapy Management Handbook - Softcover

As pharmacy practice continues to evolve with a greater focus on medication therapy management (MTM), collaborative drug therapy management is becoming increasingly important and widespread. Designed as a starting point for establishing collaborative practice, Collaborative Drug Therapy Management is intended for any pharmacist or administrator seeking to initiate, improve or expand CDTM services in their organization.

This handbook begins by addressing the core elements necessary for establishing a CDTM program. It includes in-depth example guidelines created and used in daily practice by a team of outstanding clinical pharmacy specialists at Harborview Medical Center. Case studies illustrate how to use these guidelines in practice. Chapters on credentialing and measuring outcomes provide the basis for improving and maintaining the quality of your practice.
