It is only after the consideration Of each Of these fields that a diagnosis is made; indeed in many instances it is the only safe way in which an opinion as to the mentality Of the child can be formed.

The blank is to be filled out only after the data on the pedagogical record, personal history, physical examination and psychometric test blanks have been Obtained. Practically, the Official making the psychometric tests will no doubt in cooperation with the school physician be qualified to pass judgment upon each field of inquiry. The diagnosis Should be made by the school physician or if possible by a qualified psychiatrist. The scope and the main points to be determined in each Of these fields is as follows.
