RICE is professor, department of health services, University of California, Los Angeles, School of Public Health. GERALD F. KOMINSKI is an assistant professor, department of health services and associate director, Center for Health Policy Research, University of California, Los Angeles, School of Public Health. They are the coeditors of the first edition of Changing the U.S. Health Care System (Jossey--Bass, 1998).
