Donna has held a variety of management positions in corporations and founded her own consulting firm in 1986. Sample Questions from the Team Performance Questionnaire
The questionnaire is divided into six sections. Two sample questions from each of the six sections follow. Each question is answered on a scale of 1 ("Strongly Disagree") to 5 ("Strongly Agree").
1. We are clear about the team's goals.
2. Our goals are inspiring to team members.
7. Members feel a sense of belonging to the team.
8. We are able to share information and ideas freely and honestly.
12. Each member is able to fully use his/her skills and abilities.
15. Team meetings are efficient and productive.
16. Members are able to communicate easily with one another.
19. We are able to discuss and resolve conflicts.
22. Each member demonstrates commitment to the team.
