Excerpt from Certain Forms of Infection in Typhoid Fever<br/><br/>The bacillus of typhoid fever is, therefore, distributed in such a way as to indicate that it has, at some time during the disease, been present in the blood current. Yet it must be remembered that it is not, unless most exceptionally, found in the blood outside of organs, even at autopsies. The rarity of this condition is sufficient apology for recording cases in which a general typhoid infection of the body existed at autopsy. This case presents, in addition, another feature which is perhaps of interest. The recent contributions to the study of the relation of the typhoid bacillus to local foci of suppuration have tended to show that in a variety of in?ammatory processes this organism alone is concerned. This View, first advanced by A. Fraenkel, has, as is well known, been vigorously contested by Baumgarten and E. Fraenkel. The latter have contended that other bacteria, particularly the pyo genic cocci, were present, along with the typhoid bacillus, and were overlooked or had died out at the time of the examination. Now in the kidneys in this case a number of abscesses existed in which no other micro-organisms than the typhoid bacilli were demonstrable. Any assumption that other bacteria had been present is not to be reconciled with the short duration of the illness; and the numbers and relation of the typhoid bacilli to the foci of suppuration leave no doubt of their action. The clinical course of the case was unusual. I am enabled, through the kindness of Dr. Osler, to whose ward the patient had been admitted, to give an abstract of the clinical history.
