This title offers an innovative and fun dementia assessment game! Gather essential information quickly and accurately using this colourful board game. The design builds on the familiar and is non-threatening, stimulating and fun to use. It can be used in different settings, at home, in care centres and in hospital. Carers and family members can 'play' too. It is suitable for all cultural backgrounds. How Take Shapes works. The board game assesses four skill areas - cognitive, sensorimotor, intrapersonal, and interpersonal - in one session. It is backed up by an assessment booklet with guidelines and tick boxes. The results give an immediate, holistic assessment of the patient's condition and provide the insights to develop effective, individually-tailored care plans. Carers, family and dementia sufferers are able to identify which functions are still in use and take steps to maintain them as long as possible, and to find ways to compensate for those that are diminished or lost. Crucially, communication and understanding between all parties - patients, families and carers - is improved. The need for Take Shapes. Take Shapes was developed by an occupational therapist specialising in treating dementia and stroke patients as she found that existing assessment tools made patients feel anxious and failed to provide the full range of information she needed to create effective, individually-focused care plans. Health practitioners and the government recognise the need for more effective dementia assessment and treatment. The 'Living well with dementia: A National Dementia Strategy' highlighted the need for change in dementia care. Its recommendations include putting people and their carers at the heart of planning their lives and delivering services based around the needs of individuals and families. Take Shapes helps to meet these objectives.Vom Verlag:
